Enclomiphene vs hCG: how do the two fertility-sparing testosterone options differ?

Both aim to raise a man’s own testosterone without shutting down sperm production the way testosterone therapy can. hCG is FDA-approved and acts like LH directly on the testes; enclomiphene is a pill that is not FDA-approved and is compounded. The AUA guideline says clinicians may use either class in men with low testosterone who want to keep fertility.

How do enclomiphene and hCG raise testosterone differently?

They act at different points in the same chain. Enclomiphene works in the brain, blocking estrogen feedback so the pituitary sends more LH and FSH to the testes. hCG skips the brain: its label says its action is virtually identical to LH, and it stimulates the testes’ Leydig cells to make testosterone. In enclomiphene’s phase 3 trials, LH, FSH and testosterone all rose, while testosterone gel lowered LH and FSH. Enclomiphene phase 3

Do enclomiphene and hCG protect fertility?

Both are used for that purpose. The AUA guideline says testosterone therapy should not be prescribed to men currently trying to conceive, and that clinicians may use hCG, SERMs, aromatase inhibitors or combinations of them in men with low testosterone who want to keep fertility. That is a conditional recommendation. AUA

Is enclomiphene or hCG FDA-approved?

hCG is: products such as Novarel are approved for selected cases of hypogonadotropic hypogonadism in men, undescended testes in boys, and ovulation induction. Enclomiphene is not approved; it is in FDA’s 503A Category 1, so pharmacies can compound it for an individual prescription while FDA evaluates it. Novarel label

FDA lists enclomiphene citrate in Category 1 of its 503A bulk substances list, which is an interim policy, not an approval. FDA

hCG is not approved for weight loss. FDA says hCG is not approved for use without a prescription for any purpose, and warns consumers to avoid over-the-counter hCG diet products. FDA

Which might suit whom: Enclomiphene or Human chorionic gonadotropin?

The choice depends on the cause of your low testosterone, your fertility plans and how you weigh approval status. Questions to bring:

  1. Is my low testosterone secondary, so that either option could work?
  2. Do I want children now or later, and what should we check before and during treatment?
  3. Would an FDA-approved hCG product or compounded enclomiphene fit me better, and why?
  4. How would we monitor testosterone, estrogen and blood counts on either one?
  5. Does my insurance cover hCG for my diagnosis?

DiscoverMedica does not recommend one over the other. Only a licensed clinician who knows your history can decide.

Enclomiphene vs Human chorionic gonadotropin: questions, answered

Is enclomiphene better than hCG?

They have not been compared head to head in a large trial. hCG is an FDA-approved injection for selected men with hypogonadotropic hypogonadism; enclomiphene is a pill that is not FDA-approved and has limited long-term data. The AUA guideline lists both classes as options for men with low testosterone who want to keep fertility.

Can hCG and enclomiphene be used together?

The AUA guideline says clinicians may use hCG, SERMs such as enclomiphene, aromatase inhibitors or combinations of them in men with low testosterone who want to keep fertility. Whether any combination fits you depends on your labs and goals, and it is a prescribing decision for your clinician.

Is hCG used for weight loss?

It should not be. No hCG product is approved for weight loss, and the label states that hCG has not been shown to be effective therapy for obesity. FDA says any weight lost on hCG diets comes from severe calorie restriction, not from the hormone.

What do enclomiphene and hCG cost?

Enclomiphene: Listed starting prices run from $42 to $99.99 across 4 providers, checked October 7, 2026. hCG: 1 provider lists it without a public price. Listed prices change often, and labs and visits may be extra.
